Patient Portal Development Services

We design and build the interfaces where care actually happens: patient portals with records, booking, and secure messaging, and clinician-facing apps for scheduling, notes, e-prescribing, and the doctor side of telehealth. Built to cut administrative work, integrated with your EHR, ready for a security review.

Patient Portals Patients Actually Use, and Clinician Apps That Give Time Back

Patient portal development in 2026 starts from a fact that changed the brief: the portal is no longer optional and no longer a website. According to ONC Data Brief 77 (July 2025), 65% of Americans accessed their online medical record in 2024, 57% did it through a smartphone app (up from 38% in 2020), and 51% accessed a record on behalf of someone else, more than double the 2020 figure. Among people whose provider encouraged portal use, 87% used it; among those not encouraged, 57%. The portal decides whether a practice’s digital front door works, and the encouragement decides whether patients walk through it.

We build both sides of that door. For patients: registration and identity verification, records and test results with clinical notes, appointment booking with waitlists and reminders, secure messaging, prescription refills, bill pay, and proxy access for caregivers. For clinicians: schedule and inbox management, structured and ambient clinical documentation, e-prescribing, order entry, and the provider side of video visits, designed to take administrative minutes out of the day rather than add screens to it. Every build reads from and writes to the system of record through HL7 v2 and FHIR R4; our guide to EHR integration covers what each interface costs and how long it takes.

Two rules shape the 2026 build. The 21st Century Cures Act requires that patients get electronic access to their health information without information blocking, so the portal has to expose the full record, including clinical notes, not a curated subset. And under the HHS Section 504 rule, providers receiving HHS funding must bring web and mobile content to WCAG 2.1 AA by May 11, 2027 (May 10, 2028 for organisations under 15 employees). A portal that fails an accessibility audit fails procurement.

A patient portal on read-only EHR access costs $40,000 to $100,000 and ships in 3 to 6 months. A portal plus clinician app with an admin backend and bidirectional EHR sync runs $120,000 to $250,000. Multi-site platforms with AI documentation and identity federation start at $250,000. For the regulatory lanes behind these numbers, see our healthcare app development guide.

Patient Portal and Clinician App Solutions We Build

Six product types cover most engagements. Patient-facing products are judged by adoption; clinician-facing products are judged by minutes saved per encounter. We scope and measure them differently.
01
Patient Portal Apps
Web and mobile patient portal app development in one codebase: records, test results with clinical notes, appointments, secure messaging, refills, bill pay, and proxy access for parents and caregivers. Built for the 57% of patients who now open the portal on a phone.

02
Patient Engagement Platforms
Patient engagement software development beyond the record: care plans, reminders, questionnaires, education content, outcome tracking, and outreach campaigns that lift portal use. Adoption is the metric, and we instrument it from day one.

03
Appointment Booking and Scheduling
Self-service booking against real provider availability, waitlists, intake forms, insurance eligibility checks, reminders, and no-show recovery. Works as a module inside the portal or as a standalone doctor appointment app.

Clinician Apps and Practice Management
Schedule and inbox management, task queues, referral tracking, and practice management workflows for front desk and billing. Designed around the encounter, with the fewest taps between opening the chart and closing the note.

What Goes Into a Patient Portal Build

EHR Integration
HL7 v2 feeds for ADT, scheduling, and results; FHIR R4 APIs for patient, encounter, observation, and document resources; SMART on FHIR launch for Epic, Cerner, athenahealth, and eClinicalWorks. Read-only first, bidirectional when the workflow needs it.
Identity and Proxy Access
Identity verification at enrolment to NIST 800-63A-4 guidance, MFA, session controls, and a proxy model that handles parents, adult children, and legal guardians with the right consent records and age-based transitions.
HIPAA-Ready Architecture
Encryption in transit and at rest, role-based access, immutable audit logs, breach notification workflows, and a business associate agreement with every vendor that touches protected health information. Designed to the current Security Rule and its proposed update.
Accessibility to WCAG 2.1 AA
Built and tested to the standard the HHS Section 504 rule requires by May 2027, with screen-reader support, keyboard navigation, colour contrast, and plain-language content that also lifts adoption among older patients.
Secure Messaging and Notifications
Patient-to-clinic messaging with triage queues, templated replies, and turnaround tracking; SMS, email, and push notifications with consent management and content rules that keep protected data out of unencrypted channels.
Analytics and Adoption Instrumentation
Enrolment funnels, activation rates, feature use, message volumes, and no-show rates in one dashboard, so the practice can see whether the portal is reducing calls and visits or adding to them.

Patient Portal Development: Frequently Asked Questions

How much does patient portal development cost?
A patient portal on read-only EHR access, with records, results, booking, and messaging, costs $40,000 to $100,000. Adding a clinician app, an admin backend, and bidirectional EHR sync brings the range to $120,000 to $250,000. Multi-site platforms with AI documentation, identity federation, and several EHRs start at $250,000. Budget 15% to 20% of build cost per year for maintenance and EHR version changes.
How long does it take to build a patient portal?
A first version ships in 3 to 6 months. The EHR interface sets the pace: read-only FHIR access can be live in weeks, while bidirectional sync with scheduling and orders adds 2 to 4 months for vendor certification and testing.
Should we build a custom patient portal or use the one that comes with our EHR?
Use the vendor portal if your workflow fits it and you run one EHR. Build when you span several EHRs or sites, when the vendor portal cannot expose the features your patients ask for, when adoption is low and you need control over the experience, or when the portal is part of a product you sell. Most custom builds keep the EHR as the system of record and replace only the patient-facing and clinician-facing layers.
Can the portal integrate with Epic, Cerner, or athenahealth?
Yes. We integrate through HL7 v2 feeds and FHIR R4 APIs, with SMART on FHIR where the vendor supports it. Read-only access to demographics, results, and notes is the fastest path; scheduling and order writes go through each vendor's certification process, which we plan into the timeline.
What does HIPAA require from a patient portal?
Under the current Security Rule: encryption in transit and at rest, access controls with MFA, audit logs, breach notification, and business associate agreements with every vendor handling protected health information. The proposed 2024 update would make MFA and encryption mandatory rather than addressable; we design to it now.
Do we have to make the portal accessible?
If your organisation receives HHS funding, the Section 504 rule requires web and mobile content to meet WCAG 2.1 AA by May 11, 2027 (May 10, 2028 for organisations with fewer than 15 employees). We build and test to that standard by default, because accessible portals also see higher adoption among older patients.
How do you increase patient portal adoption?
ONC data shows 87% of patients use the portal when their provider encourages them, against 57% when nobody does. We build the encouragement into the product: enrolment at check-in, results and notes released on time, proxy access for caregivers, and a mobile app, since 57% of patients now access records on a phone. The analytics dashboard shows which of these is working.
