Patient Portal Development Services
Patient Portals Patients Actually Use, and Clinician Apps That Give Time Back
Patient portal development in 2026 starts from a fact that changed the brief: the portal is no longer optional and no longer a website. According to ONC Data Brief 77 (July 2025), 65% of Americans accessed their online medical record in 2024, 57% did it through a smartphone app (up from 38% in 2020), and 51% accessed a record on behalf of someone else, more than double the 2020 figure. Among people whose provider encouraged portal use, 87% used it; among those not encouraged, 57%. The portal decides whether a practice’s digital front door works, and the encouragement decides whether patients walk through it.
We build both sides of that door. For patients: registration and identity verification, records and test results with clinical notes, appointment booking with waitlists and reminders, secure messaging, prescription refills, bill pay, and proxy access for caregivers. For clinicians: schedule and inbox management, structured and ambient clinical documentation, e-prescribing, order entry, and the provider side of video visits, designed to take administrative minutes out of the day rather than add screens to it. Every build reads from and writes to the system of record through HL7 v2 and FHIR R4; our guide to EHR integration covers what each interface costs and how long it takes.
Two rules shape the 2026 build. The 21st Century Cures Act requires that patients get electronic access to their health information without information blocking, so the portal has to expose the full record, including clinical notes, not a curated subset. And under the HHS Section 504 rule, providers receiving HHS funding must bring web and mobile content to WCAG 2.1 AA by May 11, 2027 (May 10, 2028 for organisations under 15 employees). A portal that fails an accessibility audit fails procurement.
A patient portal on read-only EHR access costs $40,000 to $100,000 and ships in 3 to 6 months. A portal plus clinician app with an admin backend and bidirectional EHR sync runs $120,000 to $250,000. Multi-site platforms with AI documentation and identity federation start at $250,000. For the regulatory lanes behind these numbers, see our healthcare app development guide.
